STUDENT RELEASE FORM

I hereby certify that work submitted to the Essex County Teen Arts Festival is my original work and that it is not copied from any published or unpublished source or from any other person.
I hereby grant the County of Essex and the Essex County Cultural & Heritage Commission permission to use my work and/or photographic or video likeness in any and all of its publications and any other medium without payment or any other consideration. I understand and agree that the County of Essex and the Essex County Cultural & Heritage Commission may display and/or reproduce my work in any or all of its publications, outreach and public relations efforts. I hereby irrevocably authorize the County of Essex to edit, alter, copy, exhibit, publish or distribute any photograph(s), video(s) or other material(s) for purposes of publicizing the County of Essex's and the Essex County Cultural & Heritage Commission’s programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness or work appears. I further waive any right to royalties or other compensation arising or related to the use of said photograph(s), video(s) or other material(s). I further waive any right to notice that said photograph(s), video(s) or other material(s) will be published or otherwise utilized. I hereby hold harmless and release and forever discharge the County of Essex from all claims, demands, and causes of action which I, or any person acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.
